INSTRUCTIONS:

1. Complete the necessary information indicated on page one, except for “TOTAL

SCORE”.

2. Read and become familiar with the seventeen teacher actions evaluated on the instrument before you proceed.

3. Proceed throughout the evaluation instrument determining the score and product for each teacher action as you go along. Write the total score on the first page. The total possible score is 67.5 points.

4. In the “SCORE” blank proceeding each item, a scoring range of 0-5 is possible, derive the score for each teacher action from the scoring instructions and indicate your score in the “SCORE” space.

5. The evaluation score which you indicate should be multiplied by the “Multiplier” in the middle column and the resulting “Product” should be written in the right hand column.

6. When determining each response observe in regard to a teacher action, list the effective and ineffective teacher behaviors in the space provided following the scoring instructions.

7. Summarize on the last page the overall effectiveness of the teacher being evaluated.
